Interview with Dr. Urmila Soman, Lead Surgeon in Robotic Gynaecology Surgeries & Senior Consultant Gynaecologist (Minimally Invasive Gynaecologic Surgery)

In the rapidly advancing world of medicine, robotic surgeries have emerged as a revolutionary approach, offering precision, minimal invasiveness and quicker recovery times. Leading this transformative journey in women’s healthcare is Dr. Urmila Soman. Known for her expertise, empathy, and commitment. Dr. Urmila Soman has not only brought state–of–the–art technology to the surgical table but also gifted countless women a new lease on life.

Let’s bust myths about the role of robots in the operation theatre, and recount the  joy of seeing patients regain their confidence and dignity.

  • Kerala recently witnessed its first robotic vaginal reconstruction surgery under your expertise. How did this milestone impact the medical community and, more importantly, the patient?

Yes, robotic surgery played a crucial role in this complex genital reconstruction surgery, marking a first in Kerala. The patient, diagnosed with MRKH- Type 2 syndrome, had no uterus and vagina, a missing kidney and displaced ovaries. Robotic technology offered the precision required for this delicate procedure, minimizing risks to her kidney and ureter, which were inside the hip bone. The advanced technique created a new gap between the bladder and rectum, essential for genital reconstruction. We used peritoneal flaps from the abdomen, which closely resemble genital skin, offering better post- operative outcomes and easier care. While the patient cannot conceive naturally due to the absence of a uterus, the surgery allows for future IVF and Surrogacy, giving her a chance to have a biological child.

What stays with me is her mother’s smile after the surgery. It wasn’t just a transformation – it was the emotional liberation she felt. That moment reminded me why I chose this profession. Medicine is not just about healing bodies; it’s about restoring lives and empowering individuals to embrace their fullest potential.

  • Robotic surgeries are often misunderstood as being entirely performed by robots. How the process actually work?

One of the common misconceptions is that the robot independently performs the surgery. Robotic surgery is completely surgeon-driven. Surgeons operate from a console equipped with highly sophisticated controls. These controls maneuver robotic arms, which execute instructions with extraordinary precision. The robot is essentially a tool, an extension of skills,allowing us to work with unparalleled accuracy, especially in intricate procedures like pelvic reconstructions or tumour removals.

The advantages are remarkable – smaller incisions, reduced blood loss, minimal post-operative pain, and faster recovery. Patients can return to their normal lives much sooner, and as a surgeon, it gives me immense satisfaction to see them thrive

“Many believe robotic surgeries are entirely controlled by robots, but that’s a myth. These procedures are 100% surgeon-controlled, with robotic technology enhancing unparalleled precision, minimal scarring, and quicker recovery. This advanced technology enables us to perform complex procedures with enhanced accuracy, ensuring the best outcomes for our patients, and more effective outcomes for patients.”

  • What makes robotic surgeries particularly advantageous in gynaecology?

Gynaecology often involves complex, delicate structures that require a high degree of accuracy. Robotic surgeries allow us to navigate these complexities effortlessly. For instance, in cases of endometriosis, where lesions are close to vital organs, or in fibroid removals where preserving fertility is crucial, robotic surgery offers unparalleled precision.

Moreover, the 3D visualization and enhanced dexterity provided by the robotic system ensure that even the most challenging procedures are safer and more effective. Patients benefit from shorter hospital stays, fewer complications, and a quicker return to their routines.

  • Should robotic surgery be used for all types of surgeries?

Robotic surgery is primarily recommended for complex or delicate procedures that require high precision, minimal invasiveness, and better outcomes. While it offers advantages like smaller incisions, faster recovery, and reduced risk of complications, not all surgeries necessarily require robotic assistance.

Traditional or laparoscopic techniques may be equally effective and cost-efficient for simpler procedures. The choice depends on the patient’s condition, the complexity of the surgery, and the surgeon’s expertise. Discussing the options with your doctor to determine the most suitable approach is always best.

  • What’s your vision for the future of robotic surgeries?

To make robotic surgeries accessible to all women, irrespective of socio-economic barriers. I believe that every woman deserves the best care possible, and robotic technology is the future of gynaecological surgeries. As more hospitals adopt this technology, I hope to mentor young surgeons and create a robust system where expertise and innovation go hand in hand.
